Uterine angioleiomyoma is a rare entity. Angioleiomyoma benign vascular tumor that originates fromthe blood vessels. Although exceptionally they can be found in the uterus, are commonly reportedin lower extremity. Preoperative diagnosis tricky. We publishing this case to highlight uterinecontractions as possible cause of peripartum rupture. The patient was her fourth pregnancy, with history three vaginal deliveries. She presented theemergency department our hospital labor pain, at 39 weeks gestation, and admitted thelabor unit active labor. had an uneventful delivery good neonatal outcome. Afterdelivery, complained abdominal shoulder tip distention. Examination revealed tender mass felt on left side uterus up hypochondriac region, contracted normal lochia. A CT (computed tomography) scan carried out remarkable for left-sided, mixed-density, abdominopelvic lesion, measuring 18×12×9 cm, moderate amount hemoperitoneum. Intraoperatively there 1 liter free blood, ruptured subserous myoma connected 20×15 cm hematoma. myomectomy out. Histopathology reported symplastic-type cells. infrequent, complicated angioleomyoma should suspected differential ofabdominopelvic hematoma patients uterine fibroids
